These changes are simply visual as your providers, services, and care will remain the same. Our priority is to make this transition as smooth as possible while continuing to provide the compassionate, high-quality support you’ve come to trust.Â

📧 Staff Email Addresses: You can reach our staff using the following email address format: first initial + last name @ fernbrook.org (for example, jsmith@fernbrook.org). If you receive an undeliverable/bounce back message, please call 320-983-2335 to verify the email address/spelling.
📞 Phone Numbers: All phone numbers are remaining the same. You may even receive text messages from our phone numbers, now!
📠Fax Number: Our fax number has changed! Please use 651-925-0337 moving forward, update your records accordingly, and discard our previous number if saved.
Client Referrals: All referrals can be sent to intake@fernbrook.org Or you can use our Online referral form HERE (This link opens our secure referral form on Fernbrook’s website as we work to combine our websites.)
